MAKE LPA ONLINE: The online service helps you create a lasting power of attorney (LPA) for England and Wales.
The digital facility explains the process and guides you through answering the questions.
When you create an LPA online you can save and print the information. You can also sign and send it to the Office of the Public Guardian (OPG) for the registration when you have finished.
Note: The online service saves the LPA as you progress with the digital application. So, there is no need or urgency to complete it in one go.
Other people will need to sign the forms no matter which version you use (online or paper). After choosing your attorney, they will also need to sign along with your witnesses.
There is no legal requirement for you to complete the application yourself. Someone else can make an LPA online or fill in the paper forms on your behalf. You can ask a family member, a trusted friend, or a professional (e.g. a solicitor).
Note: You must register a lasting power of attorney (LPA). Otherwise, your attorney will not be able to make any decisions on your behalf.
Create an Account to Make an LPA Online
Once you create an account you will be able to start making your lasting power of attorney. You will also be able to:
- Get access to further help and guidance for each step.
- Save the forms and then complete them at a later date.
- Review your answers to the questions and correct any errors.
Note: You will need to be able to print out the forms so you can sign them after finishing the online application.
Using LPA Paper Forms
You can download the lasting power of attorney forms and associated guidance notes. Print off the forms to make and register a lasting power of attorney (LPA).
Signing Lasting Power of Attorney Forms
You must sign all the forms before you return them or send them off. The other people that also need to sign the papers include:
- The attorneys
- The witnesses
- A ‘certificate provider’ (person confirming you are creating the LPA by choice and understand what you are doing)
Witnesses and Certificate Providers
Whoever you choose to be your witness and a certificate provider must be at least 18 years old. The nominated attorneys can witness each other’s signature, but they cannot:
- Witness the donor’s signature.
- Sign as the certificate provider.
Note: The person who is appointing the attorney [the donor] cannot be a witness for the same application.
